Thank you for your interest in donating to Avita Home Health & Hospice. Our mission of not turning away anyone because of an inability to pay for home health or hospice services continues – but only with your help. By participating in our fundraising efforts, you assist us in providing health care and comfort to those with an acute, chronic and/or life limiting illness and their families. There are many ways for you to give.
Honor a Loved One
You may also honor a friend or loved one with your gift to Avita Home Health & Hospice. Your donation will be acknowledged to the family by an announcement card with your name and the designation of your gift. The contribution amount will not be revealed.
Matching Gifts
Did you know many companies offer matching gift programs for employees who contribute to charitable organizations? Most of these programs match contributions dollar for dollar, while some double or triple the amount of your gift. Others even provide matching funds to support employee volunteer hours. Contact your employer’s Human Resource Department for more information.

Memorial Garden Brick
Help the Avita Home Health & Hospice Memorial Garden grow with a personalized brick/paver etched in honor or in memory of your someone special that can be placed in the walkway surrounding our gazebo. Benches and trees are still needed and can be purchased by donors as a memorial gift.
