In November We Remember Our Loved Ones Who Have Died

Avita Home Health & Hospice and the Avita Health Foundation hold our families, patients, supporters, volunteers, and staff who have died in our thoughts and prayers. In the lead up to Remembrance Month, we invite you to record the names of your family and friends who have died, so that as a community we may remember them in our prayers.

This November we will list all remembrance intentions throughout the month in the Hord Chapel at Avita Bucyrus Hospital, in the Boyd Chapel at Avita Galion Hospital, and in the Ontario Hospital Chapel. In addition, intentions will be memorialized at Avita Home Health & Hospice’s Annual Memorial Service – more details will be provided to those who submit intentions.

If you’d like to memorialize a loved one, please complete the form below. If you would like to make an additional contribution to the Rememberance Program, please visit the Avita Health Foundation online donation page. All contributions support Avita Home Health & Hospice’s Hospice and Palliative Care Programs.

For more information please contact Cathy Sapp (Executive Director of Avita Home Health & Hospice) at csapp@avitahs.org or Chuck Cangelosi (Chief Development Officer of Avita Health Foundation) at ccangelosi@avitahs.org.
