Skilled Nurses

Our registered or licensed practical nurses will come to your home to give injections, manage IVs, handle dressing changes, catheter insertions, irrigations, assessment of unstable blood pressure, pulse, or provide instruction for a new diagnosis such as diabetes, heart disease, hypertension, or heart failure. We have nurses on call and available 24 hours a day, 7 days a week to respond to questions and health concerns.

Wound Care Specialists

Our Certified Wound Care Specialists will assess the patient’s overall health including nutrition, mobility, medications and skin condition, noting any indication of infection, size, location of wound and contributing factors. They will continue to oversee the treatment of the wound, provide education to improve nutrition, decrease risk of infection, and improve mobility or position changes to reduce pressure to the skin surface.

Home Health Aides

Home Health Aides assist in caring for our patients through daily living tasks, promoting safety in the home. These services include grooming and dressing, light housekeeping as well as meal planning and preparation. We also provide short-term and intermittent bathing needs while our patients recover from a brief illness.

Physical Therapists

Physical Therapists will come to your home for rehabilitation with a therapeutic program of exercises and instruction to regain mobility following a stroke, fracture, or other injury.

Occupational Therapists

Occupational Therapists aim to promote independence and may involve an assessment of daily living skills, home visits, and adapting the home to promote independence and safety.

Speech Therapists

Speech Therapists help attain optimal levels of communications skills that are helpful after a stroke or surgery. The therapist may also help with the individual having difficulty swallowing.

Social Workers

Our social workers offer an assessment of social and emotional needs, referral to community resources, financial resource information, crisis intervention, teaching of self-management skills, as well as counseling for long-range planning and decision making.

Patient Support Volunteers

Avita Home Health & Hospice augments their professional staff with the use of specially trained volunteers. Volunteers may provide support such as assistance with errands, transportation and friendly visits.

Palliative Care

Palliative care is focused on providing patients with relief from the symptoms, pain and stress of a serious illness. Our team works with a patient’s other doctors to provide an extra layer of support. It is appropriate at any age and at any stage in a serious illness and can be provided along with curative treatment. The goal of palliative care is to provide the best possible quality of life for both the patient and their family.
